Chiswick Family Practice is seeking a dedicated, motivated, enthusiastic and forward-thinking General Practitioner to join our friendly practice team. The postholder will primarily undertake patient-facing clinical work, contributing to the delivery of safe, effective, and responsive general practice services to our registered population. The role also offers opportunities to contribute to wider practice initiatives and quality improvement activities.

Assess and triage patients, including same-day presentations, providing treatment or referrals in line with NICE guidelines and local pathways. Manage undifferentiated and complex conditions through history taking, examination, diagnosis, and care planning. Deliver face-to-face and telephone consultations, supporting long-term conditions, acute presentations, and undertaking clinical administration (prescriptions, referrals, and test results). Provide high-quality, evidence-based care and prescribe in line with national and practice policies. Support vulnerable groups, including care home residents, and identify and manage mental health needs with appropriate referral. Accurately interpret pathology results and ensure effective use of read codes. Assist with clinical emergencies, safeguarding, and medicines optimisation. Contribute to QOF and practice targets, liaise with external services, support personalised care plans, and undertake home visits where required.
